And I don’t think anyone has covered this, but if you’re paying with a credit card, they will run the card for your total amount and bring it back to you. At that point, you can add a tip, total it up, and sign it.

1If you’re paying cash, it’s highly unlikely you’ll have the precise amount of cash that you want to give, including tip. In this case, you can give more, wait for change, and make do with that. But you may need specific denominations, in which case you can ask them to break your change a certain way. or it may be as simple as giving them the total, telling them how much you want back, implied that the remainder is for them.
