@sun @subnetter @mitchconner The film "Beware! Children At Play" was pretty notorious for the mass on-screen killing of children in its finale, but it was a low-budget slasher flick and the acting is so bad that the scene is usually shared for laughs.

I think Alien vs Predator: Requiem also had an on-screen child death, and also depicted the deaths of pregnant women in a maternity ward. Pretty sure those scenes were added for deliberate shock value.
