The Painkiller was created at the Soggy Dollar Bar in the British Virgin Islands. It's a bar on a tiny ancillary island. There is no bridge access. There is no dock access. Just a small island with a bar. You have to wade over to the beach from your boat...thus the Soggy Dollar Bar.
